Community Treatment Team

The community treatment team (CTT) works with adults in the community with an acute physical need who could potentially be treated at home, rather than attend accident and emergency (A&E). The aim of the service is to prevent unnecessary hospital admissions for those people who are experiencing an acute, physical health crisis, for example: a suspected infection, falls without injuries, or an exacerbation of a long term condition such as COPD.

The service is provided to residents of Barking and Dagenham, Havering and Redbridge.

 

How does it work?

Referrals can be made by anyone - self referral can be made by patients or through their carers direct to the service. Referrals can also be made via GPs and healthcare professionals direct to the service. All calls will be triaged by a nurse or therapist before acceptance to assess if they are suitable for the service and ensure appropriate care is provided. Please note, carers/family should be present with the patient at the time of making the referral if patient cannot make the referral themselves as the nurse/therapist will have questions that require answering in the patient’s presence.

Wound Care

What is this service?

Most cuts and grazes can be treated at home and will start to heal in a few days. But some wounds may need to be treated by a medical professional if there's a risk of infection, if the injury is serious, or if you have recently had an operation.

Simple wound care services are available to all patients over the age of 16 who are registered with a GP practice in Barking & Dagenham, Havering or Redbridge.The services support patients with managing wounds such as skin ulcers, first degree burns, post-surgical sutures, clips removal and routine dressings.

How does it work?

You will be advised to book an appointment by your doctor or a nurse if you have a wound that needs to be cared for with a dressing.

Booking an appointment

You can book an appointment seven days a week at a time convenient to you. Appointments are available Monday to Friday from 8am-6.30pm and on Saturday and Sunday between 9am-5pm.

Catheter Care

A new advice line has been introduced to provide patients living with a catheter, access to care and advice to help them manage any issues they may be facing.

The advice line is for people who are:

Mobile and not housebound

Living in Barking and Dagenham, Havering and Redbridge

Have an indwelling urinary or suprapubic catheter  

Aged 18 years and over.

Call the helpline for advice with:

  • A blocked or bypassing catheter
  • A routine change of catheter
  • Changing catheter bags
  • Any other questions about your catheter.

The advice line is not for: 

  • Patients who are housebound, and meet the criteria for the District Nursing Service.
  • Patients who would normally have their catheter changed in a hospital environment i.e. for excessive bleeding.
  • Those who do not have a valid NHS number.

If you have a problem with your catheter after hours between 5pm and 9am and at weekends and Bank Holidays, please contact NHS 111 or attend A&E in an emergency.
